The Access Point,  Service Navigator,   November  2013 – April 2018

·         The Access Point serves as the centralized intake hub for community mental health Case Management, ACT T  (Assertive Community Treatment Teams).

·         The Access Point also administers CASH (Centralized Access to Supportive Housing) intake and waitlist management for assorted supportive housing within the mental health sector.

·         The Access Point functions as a high volume, fast-paced, contact center environment.

·         Processing applications for community mental health services and housing.

·         Responding to a range of telephone inquiries or drop in clients related to variety of detailed program services.

·         Conduct intake interviews for the purpose of understanding  mental health issues and fit with assorted program services.

·         Understanding and application of assorted program policies and relevant legislation such as The Mental Health Act, PHIPPA and other. Relaying decisions related to eligibility and fit with assorted services to clients and referral sources.

·         Preparation of client needs assessment summary report that goes to agencies providing client service delivery.

The Second Mile Club of Toronto,   Caregiver Support Coordinator,   part time 2012 -2013

·         The Second Mile Club provides community support services for seniors, disabled adults and their caregivers.

·         Primarily functioning as the agency Caregiver Support Coordinator.

·         Providing counselling and support for Caregivers of elderly, ill or disabled individuals.
