Request service officer

Verifying of documents such as birth, marriage and death certificates as per request of the public.
All levels of verifying should be done for the documents should be correctly given to its owner or requester.
All policies regarding the documents should be strictly follows.
Cash officer

First screen the identification of the requester. An individual can only request for the documents of his/her parents, husband/wife and children. If the requester request for his/her relative such as brother/sister, aunt/uncle, grandparents or friends the requester should have the authorization letter of the owner of document and a valid identification. Also the specimen signature of the authorization letter and identification card must also the same for the cash officer will approved the request.
Input into the system the required information of the requester and the owner of the document.
Payment will be made.
Releaser

Double checking of documents and the requester is a must.
Filling them up for the portion of the received documents. Make sure they have their signature in it.
