Eligibility
Pass the Licensure examination for Social Workers
*Implements policies, regulation and work plans and renders direct service in accordance with the functions and objectives of the agency
*Implements case managements to clients
*Conducts intake interviews and accomplishes forms that will determine the client’s eligibility for service or program
*Obtains and prepares social case studies
*Conducts home visits and collateral interviews to identify factors affecting clients adjustments
*Represent the minor in court hearing (trial, monitoring and dispositional)
*Coordinates with other rehabilitation workers in the institution for effective study, diagnosis, treatment plan and progress for each youth
*Prepares progress reports and other case related report for submission to court
*initiates case conference for proper case management
*assist in planning program of activities
*Maintains close coordination to court and other social agencies
*maintain updated recorded client’s status and progress and records of activities
*In-charge in the Scholarship and Sponsorship program for the youth
*Conducts formation and training of the scholars and their parents
*In-charge in the outreach program of the students, teachers and alumni
*Supervises the housing project (building and organization)
*Initiates conferences and dialogues for better case management
*Conducts home visits and collateral interviews
*Conducts community organization
*Handles outreach clubs for students from 4th to 6th grade
*Substitute teacher for Social Sciences for 10th graders
