– Implements policies, regulation and work plans established by the agency and renders direct service
– Implements case management to clients
– Conducts intake interview and accomplishes forms that will determine client’s eligibility for the service/program
– Obtains and prepares social case study reports
– Conducts home visits and collateral interview
– Represent minor during court hearing (trial, monitoring, dispositional, etc)
– Coordinates with other rehabilitation workers in the institution for effective study, diagnosis, treatment plan and progress for each youth
– Prepares progress reports for proper case management
– Coordinate with LGU’s for after care services and NGO’s for placement of minors with no identified custodian
– Assists for planning program of activities
– Maintains close coordination with the court and the local department of social welfare and development and other social agencies
– In-charge for scholarship and sponsorship program
– In-charge of the formations of the scholars and their parents
– In-charge of the outreach activities of the school (students, teachers, staff and alumni)
